Communication need card
The Clinical Commissioning Group has developed a card to support patients who speak limited English, or that have a disability, and require access to communication support or accessible information.

The card was designed in partnership with local people from ethnic minorities and those with sensory disabilities, together with Healthwatch Hampshire.
