Hampshire Together is being led by Hampshire Hospitals NHS Foundation Trust, the Hampshire and Isle of Wight Partnership of Clinical Commissioning Groups and West Hampshire Clinical Commissioning Group, but will involve all NHS and social care services. The centrepiece will be the construction of a brand new hospital – but the programme’s impact will reach far beyond the walls of this new building, involving GPs, mental health, community care, social care and the wider voluntary sector, as well as acute hospital based care.
The programme’s vision is to deliver a healthcare system that supports patients, their families and their carers to access the right care, in the right place, at the right time in order to keep them healthy from your home to the hospital and everything in between.
A series of online public information and engagement sessions has been scheduled to discuss the current challenges around and the potential opportunities created by the Hampshire Together programme. This is a great way to share your experiences and help to shape the future of healthcare across Hampshire.
